BY SUBMITTING THIS WORKSHEET TO THE LAW OFFICES OF MARTIN
SEIDLER, YOU ARE ENTERING INTO A BINDING CONTRACT WITH THE LAW
OFFICES OF MARTIN SEIDLER TO PREPARE A GUN TRUST AND A BILL OF
SALE FOR WHICH YOU AGREE TO PAY THE SUM OF $600.00 TO MARTIN
SEIDLER AT THE ADDRESS SHOWN ABOVE UPON THE COMPLETION OF
THE TRUST. AN NFA TRUST AND BILL OF SALE IN PDF FORMAT SUITABLE
FOR PRINTING WILL BE EMAILED TO YOU.

ALL OF THE FOLLOWING INFORMATION MUST BE COMPLETED BECAUSE
A TRUST CAN LAST SEVERAL GENERATIONS.

(If the trust will be building SBR’s (short barreled rifles), the ATF requires the name of the
maker to be engraved on the (gun) receiver. The longer the trust name the more letters
need to appear on the receiver). The shorter the better in such instance.

Example:

D. Termination. The trust created by this Article shall terminate upon the death of the
Grantor, Charlton Heston, his wife, Anna Heston, and his children, William Heston and
Alice Heston. Upon termination, the Trustee shall distribute the remaining income and
principal (assets) of this trust to the following beneficiaries in the following sequential order:

1. To the surviving children, if any, of William Heston and Alice Heston, in equal
shares, per stirpes.

2. If William Heston or Alice shall die without surviving children, then the
remaining surviving children of such beneficiaries shall take in equal shares.
If all of the beneficiaries and their children described above fail to survive the Grantor, then
all of the remaining income and principal of this trust shall be distributed to the Grantor's
heirs.
